Leadership Team for ACHIEVE in Lake County

In 2009, the Lake County General Health District was selected as 1 of 43 communities in the United States to receive funding from the Center for Disease Control and Prevention to initiate this project to improve the overall health of citizens in our community, focusing on nutrition and physical activity.
